CONTOUR™DIABETES app – User Agreement, Walgreens

Please review and consent to the sharing of your health information:
If you connect your CONTOUR™CLOUD account to your Walgreens account, the CONTOUR™DIABETES app will send Walgreens your health-related information, including blood sugar readings in units of measure and a timestamp.
Blood sugar readings with a timestamp older than 7 days from the date the app syncs with Walgreens will not be credited with Balance® Rewards points.
You must be in the CONTOUR™DIABETES app and connected to the Internet before the app can sync with Walgreens.
You can disconnect your CONTOUR™CLOUD account from Walgreens at any time by visiting the "App Connections" menu and selecting "Walgreens".
I consent to Ascensia Diabetes Care sharing my blood sugar readings with Walgreens so I can earn Balance® Rewards points.
